    Please read and confirm by checkbox the following three statements.
    ​I agree to allow Alberta Street Gallery (ASG) to display my art, publicize it, and sell it. I understand that should a piece sell, I will be paid 75% of the sales price. I also understand that ASG is not liable for theft or damage to my work unless caused directly by gallery staff, and in that case, I would be reimbursed a maximum of 75% of the sales price. I also understand that if I do not retrieve my unsold art and/or checks for sales/awards by April 30, 2021, they will become the property of ASG. 
    I am submitting work that has not been shown in a previous Alberta Street Gallery 6x6 show. 
    ​My work fits the 6x6 or 6x6x6-inch size criteria. My wall art will be wired or have a sawtooth hanger or some other distinct way to hang it on a single nail. Any 3D work will have a method to hang or will be able to sit on a flat surface, or sell with a stand included in the price.
    I will check in and deliver my work to Alberta Street Gallery, 1829 NE Alberta Street, Portland, from Feb 16-21, during gallery hours of 11am to 7pm. I agree to pay $5 entry fee for each piece I deliver. (Cash or Card). After the show, I will pick up my remaining work and any check(s) March 27-31, or I will make other arrangements with gallery staff.
